The Western Colorado Veterans Health Care System, Care Management and Social Work Service Line is currently recruiting for one (1) Post-9/11 Military 2 VA (M2VA) Case Management/Care Coordination & Integrated Case Management (CC&ICM) Program Coordinator Social Worker, located in Grand Junction, Colorado. The Social Work M2VA/CC&ICM Program Coordinator, is responsible for providing leadership to five major programs across all campuses at VA Western Colorado Health Care System and provide direction to staff responsible for care coordination and integrated case management (CC&ICM) and establish policy and procedure that ensures timely access to high quality care across the continuum. The incumbent is a supervisor for the Homeless Programs, Rural Access Network for Growth Enhancement (RANGE), Home Telehealth Services, M2VA, and leads all CC&ICM coordination across multiple clinical service lines. The incumbent works collaboratively with Executive Leadership including the Chief of Staff, Service Line Managers, and other clinical and administrative staff in planning, developing, implementing and evaluating programs to support and enhance patient care across the continuum. This includes supporting surgical services and surgical case coordination. The incumbent facilitates high quality, cost effective care for Veterans patients in multiple community community multiple settings at VA expense and enrolled in various home and community-based programs. The Social Work Program Coordinator facilitates high quality, cost effective care for Veterans patients. The incumbent develops policy and procedure to promote timely and appropriate patient-centered care and interdisciplinary communication to ensure the right care, at the right time, at the right place, and at the right cost. The Social Work Program Coordinator for M2VA and CC&ICM is accountable for managing resources, supporting patient aligned care teams (PACT), and other patient-centered initiatives, research, and maintaining standards of practice to ensure high quality care across the continuum. The incumbent assists in developing and interpreting the mission, goals, policy, and performance improvement for CC&ICM care coordination, care management, and case management services. Duties include but are not limited to: Manage, plan, implement, monitor, and evaluate the delivery of services within the CC&ICM Integrated Case Management Program for the VA Western Colorado Health Care System Develop and manage the expansion of the M2VA program and services Coordinate interdisciplinary staffing, work assignments, budget, treatment services provided, and admission criteria for all five programs under direct supervision Identify areas for improvement, gather relevant data, assess the data, develop and implement ideas for improvement, and evaluate efficacy of improvements Prepares goals for each new fiscal year and reports on outcomes regularly, at the end of the fiscal year, and as requested by the Chief, Care Management and Social Work Chief Develop a systematic plan for reviewing and appraising the effectiveness of CC&ICM the Integrated Case Management Program in accordance with VA, VISN, VACO and local facility policy and procedures Participate in program development and other special duties and projects as assigned by the Chief, Care Management and Social Work Chief Provide supervisory leadership for the RANGE programs, Home Telehealth Services, M2VA and CC&ICM Being the designated expert concerning the provision of CC&ICM case management services Assist less experienced and less knowledgeable staff with technical expertise and guidance for patients with unusual or complicated medical and mental health conditions, risk factors, and ethical or legal circumstances Provide assistance for social workers, nurses, and other staff as needed or requested Regularly address the most difficult, challenging, complex, and acute patient care needs encountered in the Medical Center treatment areas and communities served Coordinate and collaborate with the facility level case management teams to include nurse and social worker case managers Establish collaborative relationships with physicians, psychologists, pharmacists, and other disciplines to foster their involvement in the care coordination, care management, and case management of Veterans Advocate on behalf of CC&ICM services and facilitate communication with facility executive leadership on case management strengths and challenges Collaborate with internal stakeholders to assess CC&ICM composition, structure and services to identify promising practices and gaps in care coordination and case management services Identify opportunities to enhance communication, collaboration and coordination across the care continuum Work Schedule: Monday - Friday, 8:00 am to 4:30 pm Financial Disclosure Report: Not required
