This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Learn more The VA Sierra Nevada Health Care System is recruiting for a Social Worker to service in the Geri-PACT team within Ambulatory Care Service, located at the Reno VA. the Patient Aligned Care Team (PACT) is a comprehensive team which delivers primary care to Veteran patients in a longitudinal rather than episodic fashion and which has as its focus: Prevention; health promotion; coordination and chronic disease management. Clinical Functions Complete thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families Assessment of psychosocial problems that cause distress, often impacting the health condition and creating barriers to care Treatment Implement evidence based treatment modalities, provide educational classes, and/or treatment groups for Veterans and families PACT Case Management/Resource Referral Help the patient and family to understand the contributing factors to the problem(s), will discuss with them the pros and cons of possible short-term and long-term solutions, encouraging them to make positive and lasting changes to reduce stressors Educate the veteran, their families and the team of all the options available to them and will collaborate with the veteran and family on the recommendations, goals and preferred options Coordinate with VA staff and community agencies to assist in problem solving as needed Responsible for using the current social work resource file of VA and community social service programs and enhancing the content to the benefit of veterans Must serve as a liaison between Veterans and/or their families and VA and community resources in order to ensure thorough delivery of services Crisis Intervention Assesses for risk factors, including suicidal and homicidal ideation, and develops a plan to address as appropriate Education/Health Promotion and Prevention Provides education related to VA and community resources, benefits, Advance Directives/Living Wills and will refer veterans/families to the appropriate interdisciplinary team member for identified health education needs Team Participation/Consultation Actively participate as a member of the interdisciplinary treatment team and actively participate through collaboration with Veterans and family as well as interdisciplinary treatment team members in the development and implementation of treatment goals and interventions Coordination of Placement Services Facilitate action for community placements through collaboration with Veterans and their families as well as interdisciplinary treatment team members to ensure that appropriate community placements are completed in a timely manner Administrative Functions Responsible for supporting the mission, policies, and procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), and the facility Responsible for entering all veteran/family contacts in the electronic record using appropriate formats and templates as well as ensuring that the encounter information is accurate Responsible for attending all appropriate Primary Care staff meetings, Social Work staff meetings and other meetings as identified by supervisor Willing to serve on committees, work groups, and task forces at the facility as deemed appropriate by the supervisor Responsible for furthering one's own professional growth through continuing education and for ensuring continuing education appropriate to the area of assignment Responsible for participating monthly in the Social Work Ongoing Professional Practice Review and for making changes to improve or enhance practice as identified through this process Provide assistance with the orientation and learning of less experienced social workers and, if interested, to serve as a field instructor for graduate level social work students Maintain a level of productivity and quality consistent with the complexity of the assignment and consistent with Social Work standards, Joint Commission standards, and facility standards Collaborates in the performance improvement processes and complies with performance measures required by the VA for their specific population and/or program
