The Registered Nurse provides care to veterans at the Oklahoma City Veterans Affairs Health Care System (OKCVAHCS). OKCVAHCS is a Level 1b major interdisciplinary teaching facility with service-lines serving medical, surgical, and neurological inpatients and outpatients, with a Level III Emergency Department, and multiple community-based outpatient clinics (CBOC). Home-Based Primary Care is provided in the veteran's home. The RN for Home-Based primary care (HBPC) performs a variety of duties, not limited to the following: Providing direct and indirect nursing care and counseling to Veterans and families in the home and clinic setting. Providing for the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being. Executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider. Supporting organizational mission and staffing needs throughout the facility as necessary during natural, man-made, pandemic or emergency situations. Serving as a role model when functioning as team leaders or charge nurse under the supervision of the Nurse Manager or designee. Functioning as a care manager for the assigned veterans, and is responsible for: assessing and continually reassessing the needs of veterans and caregivers. Delegating and supervising the care by LPNs or vocational nurses, and home health technicians, if applicable. Traveling to veterans home to conduct patient/environment evaluations for initial admission to the HBPC program and then continued assessment of the unique nursing needs of the patient and caregiver in the home setting. Teaching the patient and caregiver effective and efficient ways of managing and delivering nursing care in the home. Monitoring the patient's changing condition, assess the care necessary, and provide the needed care in the home setting. Providing for the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider Developing patient treatment plans based on a comprehensive interdisciplinary assessment of the patient and the caregiver. Leading the weekly HBPC IDT meetings in patient presentation. Coordinating the process of care with the other HBPC team members to maximize efficiency. Performing a home safety assessment and making recommendations to improve safety in the home. Assessing and meeting the unique needs of the geriatric home patient as well as his/her caregiver or significant others. Assessing the patient's continuing need for HBPC care, and planning for HBPC discharge when indicated. Work Schedule: Shifts vary depending on the needs of the service.
