This position is located in the Mental Health Intensive Case Management (MHICM) Program service line, within VA Eastern Colorado Health Care System (ECHCS) Mental Health Services. The Registered Nurse (RN) must possess the clinical skills nursing skills within a recovery oriented context to provide treatment to Veterans with serious and persistent mental illness and addictions independently and within the community. This position will require occasional driving. The RN possesses psychiatric nursing skills and judgment, he/s provide intensive case management services to assigned Veterans, consistent with the goals and methods of MHICM program as defines in the VHA Handbook 1162.08 in addition to all VA ECHCS policies and procedures. Some of the other duties pertaining to this position are (but not limited to): Intensive case management services are characterized by assertive outreach individualized treatment and recovery planning, multidimensional orientation and intensive delivery of services; Frequent and direct contact with Veterans to provide recovery oriented care and support; Flexibility of Veteran contact (Veterans are seen in any location in the community by any MHICM interdisciplinary team member who can provide assistance) with a focus on strengthening the Veteran's ability to address and live with acute and persistent challenges, symptoms and stressors, while using community settings for teaching and modeling problems-solving skills; Provide continuity of care with a focus on community integration within the mission of the MHICM program Veterans assigned to the MHICM program have been diagnosed with serious and persistent mental illness and experience a high percentage of co-morbid medical problems and substance abuse disorders. Veterans assigned to MHICM have been identified as high utilizers of psychiatric inpatient hospital resources. MICHM Veterans have previously have not been able to achieve adequate community engagement with traditional psychiatric care. Veteran care in the MHICM program is complex. MHICM programs are based on the Assertive Community Treatment (ACT) model and treatment is highly individualized and based on the Veterans needs and goals.
