The Social Worker assigned to the Suicide Prevention Program under the Mental Health Service line. The incumbent will provide high quality mental health care and services to assigned Veteran patients enrolled in outpatient mental health care at the Mental Health Clinic, Dorn VA Medical Center in Columbia, South Carolina and/or at Community Based Outpatient Clinic (CBOC) locations associated with the Dom VAMC. The Suicide Prevention Case Manager (SPCM) assists with the implementation of the suicide prevention strategies at the WJB Dorn VAMC and its Community Based Outpatient Clinics (CBOC's) through education, monitoring and coordinating services. The SPCM works within the Suicide Prevention Program under the Mental Health Service Line. The Activities include support for the identification of Veterans at high-risk; coordinating enhanced care when indicated; and education of providers, Veterans, families and community agencies/individuals on the risk factors and warning signs for suicide and treatment options. The primary component of the program is VA case management services, designed to improve/maintain Veteran's health and mental health, enhancing their ability to remain stable, housed and community integrated.
