The position is assigned to the Social Work Service at the Madison VA in the Mental Health Outpatient Clinic. The incumbent provides a full range of social work services including assessment and treatment of complex psychosocial problems resulting from mental illness, chronic and acute illness, senescence or socioeconomic and family concerns. Approximately 10-50% of time is spent providing these services and outreach to student Veterans of University of Wisconsin-Madison and Madison College. The Social Worker in this position functions within the Mental Health Outpatient Clinic, University of Wisconsin-Madison and Madison College team as a Clinical Social Worker at the William S. Middleton VA. Additional Duties: 1. Conduct psychosocial assessments and provide psychosocial interventions/treatments with supervision to a wide variety of individuals from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds. This requires knowledge of human development and behavior (physical and psychological), and the differential influences of the environment, society, and culture. 2. Independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Incumbent must possess the knowledge and ability to implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups. 3. Assess psychiatric conditions according to the DSM-5. Incumbent performs mental health diagnostic interviews with patients referred to specialty mental health clinics. Develops treatment plans in collaboration with the Veteran/family and with the interdisciplinary treatment team within established time frames. Incumbent possesses the skills to offer psychosocial interventions and measure outcomes of interventions. 4. Assess Veterans who present with psychiatric and psychosocial emergencies. Identify high-risk patients and provide case management services in compliance with the Uniform Services Handbook and VA Suicide Prevention protocols. 5. Provide crisis intervention services, including timely and appropriate interventions to prevent harm to self or others, as well as coordination of family involvement when indicated. Consults with supervisor, psychiatrist, Suicide Prevention Coordinators and/or other team members when moderate or high risk of harm is identified. Completes suicide prevention documentation in accordance with established guidelines. 6. Assess for Military Sexual Trauma and link Veteran's to clinically indicated care. 7. Possess knowledge of Veterans education benefits and campus accommodations. 8. Serve as liaison between Madison VA and Madison College community partner. 9. Coordinate community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. 10. Establish and maintain effective therapeutic relationships with Veterans and their families. Incumbent can independently work with Veterans and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills. 11. Be able to provide interventions and duties through a variety of modalities to include: face to face visits, telephone, and video modalities. 12. Hold an understanding of cultural, age and gender diversity and adapts practice accordingly. 13. Provide consultation services to other staff regarding the psychosocial needs of Veterans and the impact of psychosocial problems on health care and adherence to treatment plan. Ability to evaluate incumbents' own practice through professional peer review, case conferences and other organizational means. Incumbent can communicate effectively, both orally and in writing, with people from varied backgrounds. 14. Hold general knowledge of Veterans' benefits and services relating to special programs, service-connected compensation, non-service-connected pension. Facilitate referrals based upon Veteran needs and eligibility. 15. Meet the needs of customers while supporting VA missions. Consistently communicate and treat customers (veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner. Incumbent can provide the customer with consistent information according to established policies and procedures. Handle conflict and problems in dealing with the consumer constructively and appropriately. Has working knowledge and skill in the use of word-processing, data management and other computer systems, especially those programs in use by VHA. 16. Contribute to the team or work area by striving to improve individual, team, or organizational performance to better serve the Veteran population. 17. The incumbent provides duties as assigned.
