This position is in Social Work and Care Transitions Service at the Central Arkansas Veterans Healthcare System (CAVHS) located in Little Rock, Arkansas. Incumbent provides comprehensive case management services to post 9/11 transitioning and established Service Members and Veterans (SM/Vs) and their families. Services are provided across the continuum of care during various phases of readjustment to civilian life. Duties include but are not limited to: Provides an array of social work interventions including high-risk screening, assessments of psychosocial functioning and needs of Veterans and families, treatment planning, and resource education & referral. Provides social work services in various healthcare settings including primary care, specialty care, inpatient and ambulatory care. Ensures safe and continuous care transitions to the Veteran's Health Administration for Veteran's and Active Duty Service Members from the Department of Defense (DOD), Military Treatment Facilities (MTF), and Warrior Transition Units (WTU). Supports Veterans, Veteran's families, and Veteran's Caregivers by coordinating health needs across agencies, sites, and unique episodes of care. Services provided focus on optimizing service delivery, improving quality of care, and enhancing both patient experiences and healthcare outcomes for the population served. Assists with the public affairs component of the Transition & Care Management (TCM) Program's national mission through outreach. Maintains compliance and monitoring of established policies/procedures, quality performance measures, agency accreditation requirements, and involvement in community/VA committees.
