The Homeless Outreach Coordinator is a Senior Social Worker and is a critical position under the Health Care for Homeless Veterans Program. The Homeless Outreach Coordinator will identify and work directly with Veterans in a variety of settings both within the outpatient clinic and in the community in shelters, drop-in centers and community housing settings. The Homeless Outreach Coordinator will work to clinically assess and engage the Veteran and link him/her to needed services with the goal of psychosocial stabilization and increased residential stability. The Homeless Outreach Coordinator partners with community agencies through info1mal outreach agreements and more fo1malized contracts and agreements. At the community level, the Homeless Outreach Coordinator attends community meetings, and partners with community agencies in strategies to end homelessness among Veterans. The Homeless Outreach Coordinator represents the VA in the community and may be asked to provide education on the specific needs of the Veteran population. The Homeless Outreach Coordinator may support or work in conjunction with other programs such as HUD-VASH and the Veterans Justice Outreach program by expanding outreach efforts to include these programs or by providing case management to Veterans in HUD-VASH. The Homeless Outreach Coordinator serves as the VA facility primary point of contact (POC) on the local Stand-Downs, participating on the planning committee and serves as key chairperson on Stand-Down subcommittees. POC assist in coordinating the involvement of other VA staff members and outlining their roles in supporting Stand Downs. The Homeless Outreach Coordinator serves as the POC for Operation New Hope assisting in the coordination of the Department of Defense surplus clothing and materials for local Stand Downs. This Position involves periodically transporting patients and requires a valid driver's license. In addition, as per Incidental Drivers Policy PS-05-80, employee's driver's record will be reviewed regular through the CA DMV Employer Pull Notice (EPN) Program. Works with non-veterans 50% of the time.
