| This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more Senior Social Workers independently provide clinical, psychological, and case management services, at an advanced practice level. Services provided help meet eligible Veterans biological, psychological, social, and environmental needs. The SUD Specialist is a Senior Social Worker for HUD-VASH. This worker is an expert with the community recovery resources available and assists Veterans in linking them to supportive environments in the areas in which they live. The SUD Specialist will provide face-to-face SUD evaluations, be a liaison between Homeless Programs, other VA SORCC programs, and community resources ensuring Veterans have coordinated and seamless transitions in care. The SUD Specialist is responsible for providing high quality case management, clinical treatment, individual and group therapy, advocacy, referrals, consultation in collaboration with other members of the interdisciplinary team. The SUD Specialist supports the Homeless Programs, Domiciliary Care for Homeless Veterans (DCHV), Residential Rehabilitation and Treatment Programs (RRTP), Grant & Per Diem (GPD), and Health Care for Homeless Veterans (HCHV), while many Veterans transition care while in HUD-VASH. The housing first and harm reduction models promote engagement with Veterans in HUD-VASH, helping improve quality of life and success in maintaining permanent housing. The SUD Specialist must be able to develop appropriate professional relationships with Veterans while providing person-centered care. Duties include, but are not limited to the following: Incumbent can expand clinical knowledge in the profession, implementing methods for measuring effectiveness of services, utilizing outcome evaluations to improve treatment services, and demonstrate the ability to design changes based on empirical findings. Demonstrates the ability to teach and mentor staff and students in the area of practice and to provide supervision for licensure or specialty certifications. Provides training for other members of the multidisciplinary treatment team in areas related to SUD assessment, life skills, and co-occurring disorders. Independently conducts intake and needs assessment of Veterans eligible for homeless services. Provides all necessary assessment functions, e.g. interviewing, psychosocial histories, and SUD assessments to aid in the development of recovery plans. Develops comprehensive recovery plans with the Veteran that addresses Veterans strengths, needs, abilities, and preferences. Works with the Veteran to realize the Veteran's goals, reassess and revise regularly. Will serve as a member of a multidisciplinary Homeless Program team. Develops relationships with community agencies, network, and have regular contact and communication. Develops and maintains documentation standards that meet the expectations and requirements of joint Commission and CARF accreditation. Maintains compliance with all regulations, policies, and procedures relevant to the population served. Provides case management while Veteran establishes independence in the community, improving quality of life. The incumbent independently identifies high-risk individuals and provides crisis intervention services. Completes Comprehensive Suicide Risk Evaluation, documents and reviews as required by policy. Completes Safety Plans when clinically indicated and maintain compliance with VA policy. Expected to function independently, exercising initiative and judgment in day-to-day activities, based on expertise accumulated through education, training, licensure, experience, and reference to relevant professional literature. Seeks consultation with clinical or administrative staff as appropriate and needed. Provides case management with home visits, meeting in the community, or in office. Attends required meetings within the community and at the VA SORCC. Organizes and conducts individual and group case management and SUD treatment, to include early interventions and supportive follow-up to ensure stability in home visits and facility-based contacts. Provides SUD support to the HCHV/VASH team as a functional member of this team, to include consultation, interventions, or accompanying other case managers on home visits, when indicated by suspicion of HCHV/VASH participant SUD use.
