The Telehealth/Care Coordination Infectious Disease Registered Nurse (RN) is responsible for managing the care of the Veterans with complex Infectious Disease/Chronic Infection care needs. Monitors, tracks, and intervenes, if necessary, in the episode of care across the continuum including access to care and care planning. The position is assigned responsibility for providing comprehensive, high quality, cost effective healthcare to the patients who are rendered care at Memphis VA. As a Telehealth/Care Coordination Infectious Disease Registered Nurse (RN), he/she will provide medical treatment and personal care service to ill and injured veterans. Duties include, but are not limited to: Executes position responsibilities that demonstrate knowledge of needs, leadership, experience, and creative approaches to management of complex care, specifically in HIV and Hepatitis C. Leadership ability to identify and resolve issues relevant to patient care. Evaluates and initiates referral to various specialist to develop coordination plan across specialties in complex Infectious Disease cases. Requires collaboration within integrated teams and across the VAMC for clinic patients served by Infectious Disease Clinical Video Telehealth and Memphis VAMC. Assures provision of high quality and safe patient care. Facilitates in decision making and coordination of care. Monitors and tracks patient outcomes. Assists Veteran with follow-up and relinkage to care, including managing and scheduling Video on Demand (VOD) appointments. Clinical duties as assigned. Collaborates with patient's health care team to positively impact patient outcomes and experiences. Identifies patients with more complex cases: Uncontrolled HIV Disease; Latent and Active Tuberculosis Treatment, Evaluation, and Follow-up; PrEP, Pulmonary Mycobacterial Infection; Complex Hepatitis C Cases Establishes and maintains relationships within the Infectious Disease service (Nursing Service, Health Administration Service (HAS), attending physicians, Fee Basis physicians, resident physicians). Works closely with providers, Nursing, and HAS, to ensure patient treatment plan is appropriately scheduled and performed. Monitors record from scheduling to completion to ensure seamless high-quality treatment is provided to patient; thereby, increasing patient safety and satisfaction. Establishes and maintains relationships with other services within the VAMC. Coordinates care for complex infectious disease cases by working with other services to develop multidisciplinary teams to provide treatment to lesions or disease processes that cannot be administered solely by the Infectious Disease service in the outpatient clinic setting. Ensures seamless, timely, and appropriate treatment is provided to Infectious Disease patients utilizing Community Care, which includes radiology studies. Acts as a liaison between community providers and Memphis VAMC Infectious Disease service to ensure community provider has all needed records to provide requested care to the Veteran. Monitors, tracks, and updates records to ensure requested care has been provided to the patient. Monitors and updates HIV Cube and HIV Registry to ensure patients remain in care and receiving timely therapy. Notifies Veteran of results and treatment plan that is formed by the Infectious Disease provider. Educates patient on diagnosis. Educates patient about required treatment modality - medications, procedures, consultations, etc. Contacts Veteran for missed appointments or if is lost to follow-up. Re-educates patient on diagnosis and recommendation for treatment. Updates records with plan of care. Serves as Veteran liaison to bridge care for current clinical patients with complex, ongoing infectious disease issues, such as intravenous drug use and/or Hepatitis C, that need additional coordination to treat and prevent infectious disease. Be willing to learn and navigate the National VA HIV Cube and local HIV registry through Pyramid Analytics.
