The Greater Los Angeles (GLA) VA Healthcare Network is recruiting for a Senior Social Worker in the Community Engagement and Reintegration Service (CERS). The Senior Social Worker is responsible for providing HUD-VASH case management to Veterans and families in the HUD-VASH program. The VA is recruiting for 6 positions at the GLA campus, 1 position in the LA Ambulatory Care Center, 1 position in the Oxnard area and 1 position in the Sepulveda area. The Senior social worker provides case management services to a special patient population that demonstrates highly complex psychosocial, health or mental health problems. Duties include, but are not limited to: Provides consultation services to other VA staff regarding the psychosocial needs of homeless Veterans and the impact of psychosocial problems on health care and adherence to treatment plans; Communicate effectively, both orally and in writing, with people from varied backgrounds; Knowledge of Veterans' benefits and services relating to special programs, service-connected compensation and non-service-connected pension; Communicates and treats customers (Veterans, their representatives, visitors, and all VA staff) in a courteous. tactful, and respectful manner; Responsible for tracking census of veterans within the program and ensure that Veteran data is captured in HOMES; Work as a field instructor with MSW student interns or clinical supervisor for unlicensed VA staff.
