The full time GS-12 Senior Social Worker serves in the Health Care for Homeless Veterans (HCHV) program of the Social Work Service Line based at the Downtown San Francisco Community Based Outpatient Clinic (CBOC) which is part of the San Francisco VA Health Care System. The Salary has been updated with the new special salary rate for Social Workers in the San Francisco area. The Health Care for Homeless Veterans (HCHV) incumbent at the Community Based Outpatient Clinic (CBOC), in Downtown San Francisco, CA, identifies and engages Veterans who are homeless or marginally housed and who underutilize VA services. The incumbent provides Veterans with a range of social work services, including assessment and referrals, clinical treatment, short-term case management, advocacy and service coordination, and linkage with VA and community services. This is accomplished in concert with other members of the CBOC interdisciplinary team and community providers. The incumbent may also participate in outreach activities that are not directly related to homeless Veterans but to Veterans in general to provide them with information related to services and benefits available at SFVAHCS. The incumbent possesses knowledge and is sensitive to the needs of the homeless population. Has working knowledge and experience in the use of medical, mental health, and substance abuse treatment modalities. Is able to complete a comprehensive psychosocial assessment to identify strengths, needs, social support systems, disabilities, and barriers to accessing housing and available treatment options. Has general knowledge of Veteran's benefits and services elated to special programs, service-connected compensation, non-service connected pensions, and benefits that are available on the federal and state levels, and through city/county social services. The incumbent is sensitive to the needs of homeless Veterans and their families, realizing that homelessness creates additional stress for Veterans and their loved ones. The incumbent is aware of the special challenges in working with Veterans with co-morbid physical, mental health and substance use diagnoses, and is able to work with this Veteran population. Incumbent is responsible for outreach to homeless Veterans and community providers - is able to work confidently with city/county agencies to increase resources for "on-the-street" or marginally housed Veterans. The goal is to increase stability in Veteran's lives and assist Veterans in returning to independent, healthy, productive lifestyles. Duties also include participating as a member of the Health Care for Homeless Veterans team, which includes assisting with screening, admissions and case management needs for Veterans presenting at the San Francisco Downtown CBOC. The incumbent will act as a liaison to one of three Contracted Residential Facilities in San Francisco. The incumbent will also be called upon from time to time to fill in as the Lead Social Worker at the San Francisco Downtown CBOC.
