The Vice President - Care Management and Patient & Family Services is a tranformational appointment that supports the SVP of Value & Payer Relations and the Chief Operating Officer by providing strategic, administrative and management leadership in planning, directing, and implementing the activities and operations associated with enterprise-wide care management and care coordination, including, but not limited to: 1) strategic planning and process improvement activities to optimize integrated care management across the enterprise; inpatient, ambulatory, and community 2) leading a wide range of organization-wide transformation initiatives and special projects i.e. Enterprise Complex Care Coordination and 3) the departments of Care Management, Social Services, Child Life, Spiritual Care, Interpreter Services, Center for Families, Hospital School Program, Music Therapy, Project RISE, and other designated Community and Patient Family Services. Within these functional areas, the Vice-President ensures efficient and quality professional services designed to meet the needs of patients, personnel, and medical staff as well as the hospital’s strategic objectives. The Vice President is responsible for developing and implementing the philosophy, goals, and objectives for the care management/care coordination functions across settings of care that meet clinical, financial, and regulatory requirements of the organization and the oversight and coordination of all aspects of psychosocial care delivery, including resource allocation, strategic planning, financials, marketing activities, facilities planning and direct personnel management which includes matrix reporting. Position assures that effective, safe, cost-efficient, patient centered care is delivered to meet patient/family health care needs in a timely manner while providing and promoting service excellence. Position also ensures effective integration of patients and families into hospital strategy via oversight of patient or family care councils and ensuring their inclusion in quality and service delivery efforts. The Vice President will foster a strong customer service focus and support an effective team culture. The Vice-President will ensure promotion of an accountability based and multidisciplinary team work environment, development of business and marketing plans; leadership for key committee functions and collaboration with executive leadership, medical staff and referral centers to develop and promote CMH services and programs. |
