Columbia is the geographic center of South Carolina, with easy access to mountains (two hours west) and nationally recognized beach resorts (two hours east). The RN Mental Health Intensive Case Manager demonstrates clinical expertise, leadership, and creative approaches to management of complex care for seriously ill mental health clients within a 50 miles radius of the Medical Center or 65 miles from CBOC Range. In addition, applies clinical skills of case management which emphasizes a collaborative process that assesses, advocates, plans, implements, coordinates, monitors and evaluates health care options and services so that they meet the needs of the individual patient. Intensive acute case management is geared towards Veterans with the highest level of need. These Veterans may be hospitalized or have had recent, extended or repeat hospitalizations or recurrent episodes of crisis.
