Summary
Under general administrative direction, coordinates referrals from physicians, case management, and health care facilities for patients. /Responds to and performs pre-admission screening for patients referred to the rehab program. Provides a significant level of patient education related to illness and rehabilitation recovery process. Responsible for guiding patients with rehabilitation DRGs through the discharge rehabilitation care continuum. Responsible for the development and implementation of referral program as a means to promote the feature and benefits of the rehab program to patients, families and the community. Collaborates among all disciplines to ensure that patient�s receive care that consistently follows evidenced based standards. Completes established competencies for the position within designated introductory period. Other related duties as assigned.

Qualifications:
Education
Required: Graduate of an accredited School of Nursing Preferred: Academic degree in nursing (Bachelors or Masters degree)
Certifications
Required: Possession of current Texas State License in Clinical Specialty,BLS-Obtained through approved American Heart Association Training Center or Military Training Network.
Experience
Minimum Experience: 2 years of clinical experience in health care environment, preferably in hospital . Possess strong interpersonal skills and experience working with adult rehab patients and geriatric disabled population.
Preferred Experience: Marketing experience. Working knowledge of computers and basic software applications used in job functions such as: PC, LAN, Allscripts, E-mail, Microsoft Office, ACCESS, Printers, Fax and other machines.
