Health Care for Homeless Veterans (HCHV) is an intensive case management program for Chronically Homeless Veterans aimed at conducting street outreach to Veterans who are either homeless or experiencing housing instability and substance use disorder, and need case management assistance and psychotherapy to end their homelessness and harm reduction of substance abuse, and connect to community based and VA-supported housing services, health care and substance abuse disorder services.
