Position Summary This nurse will screen, and/or meet with patients for appropriate criteria for admissions to the Post-Acute Service Line including but not limited to Home Health, Hospice, Skilled Nursing, Long Term Care, and Subacute. Additionally, this nurse will facilitate strong relationships with local physicians and providers. Acting as the representative of the Post-Acute Service lines this nurse will travel to local health care facilities to meet the needs of potential patients.
Primary Duties ï¿½ Establishes the patientï¿½s eligibility for care ï¿½ Communicates with the families of patients ï¿½ Interacts with a variety of hospital staff from different departments ï¿½ Reviews patientsï¿½ medical records ï¿½ Explains care options to patients and their families ï¿½ Coordinates communication between patients and their physicians, case managers and other medical staff ï¿½ Develops relationships in the community on behalf of the medical facility ï¿½ Schedules speaking engagements at community events ï¿½ Works with the facilityï¿½s marketing and sales team to bring awareness of the facilityï¿½s strengths and services into the community at large ï¿½ Ability to travel within assigned territory and to sales meetings
Required Knowledge, Skills and Abilities ï¿½ Must demonstrate exceptional verbal and written communication skills ï¿½ Strong interpersonal skills ï¿½ Must be customer service oriented ï¿½ Must have excellent negotiation and persuasion skills ï¿½ Experience working in the healthcare industry ï¿½ Must be an active listener ï¿½ Must have excellent computer skills and knowledge of databases ï¿½ Must have knowledge of electronic health records ï¿½ Knowledge of medical terminology and coding ï¿½ Must have excellent organizational skills and be an effective time manager and multi-tasker ï¿½ Knowledge of skilled nursing facilities, assisted living, physician practices, hospital case management and discharge planning services ï¿½ Ability to effectively communicate Home Health, Hospice, Skilled Nursing, Long Term Care, and Subacute services at all levels of healthcare
Minimum Education/Certification/Experience: ï¿½ Current CA nursing license ï¿½ AHA BLS ï¿½ One year preferred experience in acute or long term care
