Florida Health Care Plans is seeking a full-time Licensed Clinical Social Worker with 5 years’ experience in SPMI, MAT and Crisis Intervention to join our Behavioral Health Team in Holly Hill.
Duties and Responsibilities
Responsible for providing crisis intervention services to those experiencing a mental health emergency via phone, in-home, hospital, emergency intervention and follow-up services.
Complete required documentation in a timely and legible manner consistent with appropriate crisis service documentation standards.
Provide emergency services to facilitate voluntary and involuntary hospitalizations.
Provide mental health assessments and emergency consultations as requested.
Perform independently other duties as assigned.
Strong knowledge regarding Social Services and Case Management processes in preadmission, emergency department, outpatient/ambulatory services, and hospital acute short-term care, as well as post-acute care services.
Is responsible for providing crisis intervention and/or community linkage for patients and families who are experiencing significant emotional, social, environmental, or financial stress due to hospitalization, acute, chronic, or terminal illness and/or who need help in meeting their continuing care needs.
Evaluates all Social Service referrals and identifies discharge planning needs, to include documentation of the patient discharge assessment in accordance with established guidelines and or hospital polices and processes; services to patients in the hospital acute care setting, outpatient services, and outpatient/emergency department.
Systematically gathers pertinent psychosocial data for high risk hospital inpatients whereby patient and family psychosocial needs can be identified, and an appropriate discharge plan can be developed.
Coordinates with physicians, case management staff, and other clinical staff in assisting patients and their families in understanding, accepting, and following medical recommendations. Identifies barriers to continuity of care and negotiates with insurances/third party payers to offer the patient appropriate level of care and services.
Facilitating groups within the department focused on substance abuse, relapse prevention and mental health stability.
Computer skills and experience with Microsoft Office programs
Verbal and written skills necessary to effectively communicate with various members of the health care team, other health facilities, community health related organizations, various external parties and regulatory agencies
Master’s degree in Social Work from an accredited university
Driver’s license required
Florida Licensed clinical social worker (LCSW)
Population health experience preferred
5 years of relevant work experience preferred
