Plans, organizes and implements all social work, discharge planning and case management services provided to residents and families in the Benefis Extended Care Center and Transitional Care Unit. Services will be provided in accordance with departmental and hospital wide policies and procedures.
DUTIES AND RESPONSIBILITIES:
Knowledge of hospital and departmental procedures.
Knowledge of principles of social work, interviewing, assessment, conflict resolution and problem solving.
Responsible to perform psychosocial assessments on all BECC/TCU residents to determine social service and discharge planning needs; always documents assessments in a timely manner.
Responsible to perform MDS assessments on all BECC/TCU residents and care plan accordingly; must ensure that these are completed within the timeframes set by CMS.
Performs all aspects of resident care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Knowledge of and/or ability to learn the disease processes and injuries that typically have the ability to interfere with the rehabilitation of the resident's normal functioning and ability to reach their highest quality of life. Understanding of how to work with resident's regarding these issues to provide for their psychosocial wellbeing.
Maintains current knowledge of resources available within the community, maintains supply of resource materials to be distributed to residents when needed. Is able to obtain other resources as needed.
Knowledge of medical terminology as it relates to rehabilitator and geriatric medicine.
Ability to communicate with physician regarding social service and discharge planning needs of the. Assists the physician to facilitate care upon discharge from BECC/TCU.
Communicates with family members and caretakers regarding the needs of the resident and anticipated plans.
Treats residents and families with respect and dignity. Identifies and addresses psychosocial needs of resident and family.
Interacts professionally with residents and families while involving both residents and families in the formation of the plan of care.
Maintains and respects confidentiality of resident/physician/personnel information.
Demonstrates an ability to be flexible, organized and function under stressful situations.
Demonstrates an ability to tactfully and quickly manage and resolve conflict between residents, families and staff.
Maintains a good working relationship both within the department and with other departments.
Provide education and support (both formally and informally) to residents, families and staff members on Resident Rights, dignity and quality of life issues for our residents.
Accurately determines type of assistance/setting/resources necessary for the resident and provides appropriate resources/assistance/list of facilities.
Documentation meets current standards and policies.
Collaborates with the Business Office and onsite eligibility worker, where require such assistance.
Knowledge of financial issues and the ability to assist residents and families with items such as Medicare, Medicaid, VA, etc. as it relates to their stay at TCU/BECC.
Consults other departments as appropriate to collaborate on patient care and performance improvement activities.
Maintains positive relationships with community agencies, particularly with the Ombudsman program and Adult Protection Services.
An ability to direct and organize staff conferences in relation to a case management focus.
An ability to develop and run group services for patients and families.
An ability to work in a multidisciplinary manner.
An ability to work in a culturally competent manner with residents and families from varying age, gender, race, sexual orientation and cultural backgrounds.
Exhibits genuine interest in and appreciation of adults in the later years of life.
Demonstrates the ability to deal with pressure to meet deadlines, to be accurate, and to handle constantly changing situations.
Demonstrates the ability to deal with a variety of people, deal with stressful situations, and handle conflict.

Bachelor's Degree in Social Work or related human services field.
Minimum of one year previous experience in social work or related capacity.
