Performs Case Management activities to assure necessity of patient admissions and appropriateness of continued stay. Monitors utilization of resources of all patients reviewed. Maintains confidential communication with physicians and multi-disciplinary personnel. Responsible for coordinating the delivery of services for a select group of patients by providing assistance and support to appropriate members of the health care team. The case manager expedites value-added interventions to achieve desired outcomes, and assists in collaboration, research, development, implementation, revision and reporting of case management initiatives

Minimum Education / Licensure / Certifications
Graduation from Nursing or Medical School
For Nursing School graduates, Registered Nurse license
Previous utilization, discharge planning and Case management including exposure to Interqual criteria.
