The unique location, welcoming community and ideal climate make this area the perfect place to work, live and raise a family.The Director of Case Management will report to the Chief Nursing Executive and will lead a collegial and dedicated team of case managers and social workers in their mission to provide high quality care and support to patients. This dynamic new leader will have the opportunity to develop and maintain policies, procedures and standards for Case Management, implement admission and discharge planning and support patient transitions to ensure that quality, appropriate services are delivered.This new, hands-on director will also enjoy creating and nurturing relationships with internal and external customers of the District while working closely with the Leadership Team, Social Services and the District’s Business Office to optimize the provision of care, services and reimbursement for the District.The successful candidate will bring a passion for patient care, a Bachelors Degree in Nursing, five years of progressive leadership experience in Case Management as well as a thorough knowledge of CMS requirements.
