Whole Person Care Coordinator(30-hour per week, benefited position, funded by a five-year grant)The Solano Coalition for Better Health is recruiting for a Whole Person Care Coordinator to lead integration efforts on a five-year federally funded grant to provide comprehensive care to high utilizers of multiple health care systems. This is a 30 hour benefited position through December 2020 with the possibility of increasing to full time next year.This position will coordinate care among medical, behavioral health, and social services practitioners to ensure services provided are integrated, individualized, streamlined and participant-driven. Team members consist of staff from the County, hospitals, clinics, and Solano Coalition for Better Health.
This position prefers possession of a valid license, or license eligibility, by the State of California Board of Behavioral Science Examiners as a Licensed Clinical Social Worker (LCSW), Marriage Family Therapist (MFT), Licensed Professional Clinical Counselor (LPCC), Registered Nurse, or valid license, or registration by the California Board of Psychology as a Psychologist is strongly preferred.
