Position Summary: Provides leadership for enterprise-wide Care Transition activities to drive improvements in patient outcomes and system metrics. Responsible for the planning, direction and implementation of roles, process and programs for care transition teams, including Outpatient Case Management including ACO Population Health Nurses, HIV Case Management, Specialty Care and Bundles, Transitions of Care, Home Health & Hospice and other similar future initiatives of Confluence Health. Develops post-acute care coordination strategies to reduce unnecessary inpatient readmissions and emergency room utilization. Ensures compliance with CMS Conditions of Participation for Home Health and Hospice Agency. Responsible for quality metrics to include but not be limited to: Hospital Readmission Rate, Emergency Department Utilization, Medical Loss Ratio, Annual Wellness Visit rates, HCC scores, Specialty Bundles, Post-Acute Care and Patient Satisfaction as well as required regulatory and accreditation practices. Works collaboratively with physicians, administration, colleagues and staff to achieve the goals of Confluence Health. Reports to SVP, Quality and Care Transitions.
Strategic Planning & Leadership: Works with CH senior leadership team and other leaders across the organization to implement and maintain strategic plans and activities that meet and align with the needs and goals of CH. Leads transitions of care teams to achieve department goals as well as the goals of the overall strategic plans of the CH. Keeps current with the latest emerging healthcare issues and participates in all relevant discussion forums & planning events.
Staff and Department Management: Responsible for the development, engagement, and alignment of leaders and staff to meet targeted outcomes. Accountable for setting and clarifying expectations and linking organizational communication, goals and strategy to their teams. Directs the work of care transition leaders and mentors as appropriate. Responsible for succession planning, mentoring and developing managers and directors in conjunction with the Senior Leadership Team. Conducts performance evaluations and recommends increases, promotions and disciplinary actions. In partnership with dyad partners and department leaders, develops departmental goals and plans designed to attain agreed upon goals. Develops policies and procedures affecting the department and audit procedures to determine need for change/modifications. Monitors and continually improve staff utilization to determine effectiveness and outcomes measures. Works closely with the Medical Director Value Based Care.
Operations Management: Responsible for developing operational and capital 3-5-year plans. Responsible for the operational results of the teams including patient/customer service, financial and performance metrics and projects assigned. Assists with planning of acquisition of capital and creation of operation budget to effectively and efficiently achieve departments goals and objectives including the development and implementation of new services. Coordinates the development of all operational activities in responsible departments to ensure that service levels are met or exceeded. Communicates priorities and interaction with all CH users and other to ensure the timely achievement of operational requirements, as directed by the strategic plan.
Customer Service, Advisory/Communication: Provides proactive advice, counsel and services to management and employees. Prepares and presents performance measurements to management and executives. Participates in key health system departmental meetings to improve communications and align service expectations with current plans. Provides the best possible customer service. Explains and communicates Health System and care transition philosophy, strategic goals, policies and standards to employees, medical staff, hospital administration and others across the health system and the broader system of care in the region.
Projects: Manages projects as assigned and is responsible for successful completion within budget.
Budgeting: Works with the SVP, Quality and Care Transitions to develop and proactively manage budgets and business plans and monitors performance as necessary and appropriate. Approves expenditures against the budget. Responsible for budgets of new projects under administrative responsibilities. Works closely with the SVP, Quality and Care Transitions to monitor financial performance and addresses variances. Establishes and maintains programs for systematic, continuous evaluation of and utilization of department resources.
Professional Development: Participates in professional development activities and maintains professional affiliations. Keeps abreast of developments in the field and arranges staff training programs to improve and/or enhance skills & techniques.
Works closely to ensure that effective teamwork and communication is always in place. Works to eliminate any walls and barriers throughout the organization.
Establishes good collaborative working relationships with all physicians, administrators, managers & staff and strives to provide the highest value care, in a way which also maximizes the patient/family experience.

Masters’ degree in Healthcare Administration, Nursing, Business, or related field.
7 years management experience with 5 years senior level administrative experience in a healthcare setting involving both inpatient and outpatient management responsibilities.
Internal candidates: Master’s degree with 6 years management experience with 3 years of senior level administrative experience at Confluence Health. Completion of CHVS certification.
Demonstrated expertise with LEAN methodology.
Ability to envision the future and develop strategic plans based upon those visions, and then to take those strategic plans which align with overall system strategic goals and assist in translating them into realistic tactical plans.
Proven ability to direct and manage a professional staff of 15 people including supervisory level staff.
Demonstrated ability in building positive relationships.
High levels of problem-solving ability to proactively determine problem areas and coordinate implementation of effective solutions.
Solid project management skills with an ability to coordinate many concurrent projects.
A solid understanding of healthcare business information needs.
Excellent written & verbal communication skills, and outstanding customer service skills.
Must obtain CHVS Leader Certification within one year of hire or promotion.
Must maintain primary residence in the Confluence Health service area.
Hold a valid Washington State Registered Nursing License.
