Your Role:
Applies the nursing process to plan and manage the care of complex patients in a comprehensive manner. Assists in developing and meeting key Pillar outcomes and system improvement goals including financial, satisfaction, and clinical as the nursing component of the case management triad model. Facilitates appropriate follow up care and/or refers patients (within the designated patient population) to the appropriate next level or site of care. Serves in an advocacy role on behalf of patients, families, and caregivers to ensure safe, equitable healthcare provision. Ensures evidence-based care and continuously monitors patient outcomes. Furthers professional growth by engaging in learning activities according Board of Nursing standards.
Qualifications :
Graduate of an approved discipline specific program (or equivalent experience) and 1 year relevant experience
Licensure, Certification, and/or Registration (LCR):
Position Shift : Days-Monday-Friday 8:00 AM-5:00 PM
