In collaboration with the hospital-based leader, this professional manages a team of professional staff, composed primarily of nurses and social workers. This team is responsible for providing care management services to the hospital's patient population that consists of care coordination, discharge planning, medical necessity reviews, and psycho-social services. This team also works in collaboration with the medical and nursing staff to manage length of stay and the care of patients at risk for readmission. Under leadership guidance, the Manager, Clinical Effectiveness and Care Management leads the team in continuous process improvement activities to ensure that patients receive cost-effective, high quality, and safe care. This professional also collaborates with both hospital and care management leadership to create and achieve strategic goals and initiatives. This includes supporting the tracking, trending, and reporting of department metrics that reflect the hospital's financial performance and progress toward patient satisfaction goals. As a member of the leadership team, the incumbent serves as a role model for the Norton Healthcare Service Basics that support service, communication, teamwork and safety.
Three years clinical experience in medical surgical, or critical care nursing and One year care management experience
