Manager Care Progression
Provides leadership support and oversight of entity specific care coordination and maintains close collaboration with the System Director of Care Progression. Maintains unit or geographic specific responsibility of care coordination while providing leadership to other Care Coordinators. Responsible for ensuring quality patient outcomes and overcoming obstacles to delivery of care through interdisciplinary coordination and communication. This position coordinates the interdisciplinary team and partners with physicians to ensure patient treatment plan progress. The Care Coordinator will ensure that an appropriate plan of care is initiated and will monitor care progression to ensure quality outcomes and effective transitions of care.
Bachelor's Degree (Required)
Basic Life Support - American Heart Association or Red Cross, Registered Nurse - Various
