Cooley Dickinson is currently hiring a Director of Case Management and Social Work to be responsible for provide clinical and administrative leadership to our Case Management, Care Management and Social Work programs. This includes community care management, inpatient care management and pastoral care. The Director will develop and lead care management teams towards goals of reduced hospitalizations, reduced readmissions, improved coordination, and improved patient satisfaction and outcomes.
The ideal candidate will possess a BSN, MSW or RN/BSW degrees; Massachusetts RN or Social Work licensure (or eligibility); at least five years experience in clinical care management, three years experience in a management/supervisory role, and one year adult acute care experience; a track record of success in clinical management and program development; and excellent interpersonal, communication, time management and organizational skills.
