Director of Care Coordination
Reporting to the Executive Director of Care Coordination and Special Strategic Projects, the Director is responsible for the efficient management of Care Coordination services provided at all campuses of RML Specialty Hospital (RML). This individual is responsible for all aspects of the department’s operations, including:
Management of human and fiscal resources;
Coordination of patient care (through the continuum of care) and the discharge planning process;
Utilization management and patient financial needs (including the filing of appeals of denied payment from payers); and,
Compliance with regulatory standards related to the services provided by Care Coordination, ensuring that one standard of care coordination is provided to all patients at all campuses, and ensuring that age and developmentally-appropriate services are delivered to the adult and geriatric patient populations.
In addition, this individual may, on a limited-exception-basis only, provide direct Care Coordination services to patients and/or complete Utilization Reviews.
Travel between RML Chicago and Hinsdale campuses is required on a weekly basis.

Analytic ability to assess patient/family needs and develop and execute individualized goals.
Interpersonal skills necessary to interact with patients, families, medical personnel, and other members of the healthcare team.
Ability to think critically, perform efficiently and effectively, and meet time deadlines, under the pressures of stressful or emergency situations.
Ability to direct and develop a multi-disciplinary team approach to patient-centered care.
Ability to manage and develop a team in an environment that supports staff retention.
Data analytics acumen, high attention to detail, and Microsoft Office skills (including basic knowledge of Word and advanced knowledge of Excel).
Department management experience.
Five years’ experience in utilization management, discharge planning, and/or other comparable role incorporating patient management across the care continuum (within an acute care setting and working with medically-complex adult patients).
Demonstrated knowledge of relevant local, state, federal, and independent organizations’ policies, laws, and regulations (including The Joint Commission, IDPH, and CMS).
General understanding of multi-disciplinary approach to Care Coordination and performance improvement.
Illinois Registered Nurse or Licensed Worker.
Bachelors Degree in Nursing or Social Work (from an accredited school) or Master's Degree.
Familiarity with use of Meditech (EMR)
