Responsible for screening new admissions for status/level of care and for assisting with out of network patient transfers. Works with all members of the healthcare team to assure a collaborative approach is maintained in care and treatment of the patient.
Qualifications: CA RN License. Min. 3 yrs. prior Case Management experience. Clinical Documentation Improvement certification & Interqual competency (preferred). CCM or ACM (Preferred).
