We are recruiting for a Liaison to join Lacuna Health ; this position will support our three Kindred Hospitals in the area: Westminster, Rancho Cucamonga, and Santa Ana. Flexible travel commute is needed.
The Lacuna Health Liaison is a hospital based member of the team. The liaison works in conjunction with the centralized discharge placement team and the hospital case management team to facilitate a smooth transition to discharge from our Long Term Care Acute Hospital. The liaison effectively communicates with both placement and case management and is pivotal in the discharge process. This position is a non-sales position or marketing position.
Acts as the main communication channel between Lacuna Health Placement Team and Facility.
Participates in IDT meetings daily at Facility level and communicates appropriate updates back to Lacuna Placement Team.
Closes gaps in communication and escalates issues at appropriate time and through appropriate channels.
Acts as a decision point for critical points of the Lacuna Placement Team and facility workflow.
Communication - Establishes a consistent rapport with both internal facility staff and Lacuna Placement Team. Closes gaps in communication and escalates issues at appropriate time and through appropriate channels. Reports and relays information within designated timeframes to both facility staff and Lacuna Placement Team. Ensures consistency and uniformity in process from patient to patient as closely as possible.
IDT Meetings - Actively participates in all IDT team meetings and Care Transitions meetings at facility level. Participates in scheduled Lacuna Placement Team meetings. Works closely with Case Manager at facility to ensure appropriate information is conveyed to patient and family members.
Escalation - Establishes professional and appropriate repertoire with facility team and Lacuna Placement Team to create escalation network. Escalates designated issues at appropriate time and to appropriate personnel. Identifies and addresses issues through to resolution.
Leadership - Maintains a positive environment and encourages questions, clarifications, and an open approach to the Placement Model. Possessed ability to encourage a collaborative work environment between both teams for a smooth patient experience and transition.
Must have excellent interpersonal, communication, and organizational skills.
Ability to escalate issues and see through to resolution point.
Ability to handle large patient volume with scheduled tasks and deadlines.
Proficient presentation, facilitation, and public speaking skills for IDT meetings.
Proficient user knowledge of Windows Office programs (Word, Excel, PowerPoint), and the ability to learn specialized computer applications (Protouch, Ensocare, Point Click Care, HPAS, Meditech) that are specific to handling job requirements.
Ability to develop professional working relationships and communicate effectively as a team leader
Ability to plan, organize and prioritize multiple tasks that need to be completed on a daily basis.
Ability to maintain confidentiality.
Ability to work independently. Must be self-motivated and goal oriented.
Ability to communicate effectively, both orally and in writing.
Ability to maintain regular attendance.

Clinical License or Medical Background preferred
Active LPN License Preferred
3 years of experience in Social Work or Discharge Planning highly preferred.
