The Vice President, Care Management is responsible developing a clear system-wide vision and strategy for care management across the continuum.
JOB DUTIES AND RESPONSIBILITIES
Develops, implements, and evaluates an integrated, patient-centered care management model across SLUHN.
Establishes care management goals, strategies, and objectives which support population health management and the transition to emerging payment models.
Collaborates with leadership, medical directors, and care managers across all disciplines to assure care is clinically integrated across ambulatory, acute, and post-acute settings.
Develops robust risk stratification to assure appropriate focus and utilization of resources.
Adopts and standardizes best practices to identify and impact patients at risk for high utilization of health care inclusive of health promotion and risk reduction, chronic care coordination, complex care coordination, and acute care management.
Partners with medical leadership counterpart to integrate evidence-based clinical guidelines and protocols into care coordination processes.
Creates a care management performance dashboard in collaboration with network quality leadership with a goal of achieving top decile performance in quality and efficiency metrics.
Presents reports that demonstrate outcomes to St. Luke’s Care and SLUHN leadership, and external partners.
Participates in the identification and development of high-value external partnerships that support quality and efficiency goals.
Participates in the transformational initiative related to the Patient Centered Medical Home and Specialty Practices model.
Collaborates with health insurance purchasers to evaluate and improve the health of attributed lives.
Establishes systems to ensure patient engagement in disease and care management.
Leads change management, training, and workflow integration to change culture.
PHYSICAL AND SENSORY REQUIREMENTS:
Requires sitting for up to eight hours per day, including extensive use of computer terminal and telephone. Also requires standing for presentations, driving to client locations, and some regional travel. Occasionally requires lifting of large boxes and carrying supplies to presentations. Requires hearing as it relates to normal conversation and answering questions in large groups. Requires seeing as it relates to general near and far vision.
Graduate of an accredited school of nursing required. Masters degree required. Currently licensed as a Registered Nurse in the state of Pennsylvania or New Jersey.
TRAINING AND EXPERIENCE:
8 - 10 years experience in health care required, preferably in both acute and ambulatory settings.
5 years experience in leadership in health care required.
3 years experience in managed care or population health preferred.
Solid understanding of quality improvement, organizational vision and values, interrelationship skills, as well as organizational, analytical, financial, and project management skills.
