The Case Manager is responsible for the collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual's and family's comprehensive health needs through communication and available resources to promote quality cost-effective outcomes. This includes leading the interdisciplinary team to identify problems, set goals, and develop a plan of care.
Current RN or LPN licensure from an accredited school of nursing and/or a bachelor's degree from an accredited university in nursing, social work, or a closely related medical or human services field of study preferred.
Certifications: CCM, CRRN or CBIS preferred.
3-5 years of experience in case management / coordination and providing leadership to interdisciplinary treatment teams preferred
Working knowledge of traumatic (acquired) brain and spinal cord injury and its physical, cognitive, functional and behavioral implications.
A high level of executive functioning skills including planning, organizing, implementing / executing, coordinating, managing time and resources, etc.
Excellent verbal and written communication skills.
Computer proficiency required.
Must be of good moral character.
