Gaylord Specialty Healthcare is a premier 137-bed, specialty hospital located in Wallingford, CT. We current have a career full-time opportunity available for a Workers' Compensation Nurse Liaison. Reporting to the Director of Care Management & Admissions and the Director of Business Development, the Worker's Compensation Nurse Liaison coordinates all health care services required for an organized, multi-disciplinary team approach to the management of designated patient populations admitted under Workers' Compensation Insurance; facilitates the admission of the patient from the acute care hospital or referral source to Gaylord Hospital; initiates and revises individualized care plans as indicated by assessment and patient response to treatment; assesses quality, cost-efficient care to ensure patient's plan of care promotes a safe and timely discharge; evaluates overall plan for effectiveness; involves the patient and family in the formulation of goals; monitors appropriateness of admissions and continued stay and facilitates the discharge planning with the patient, family and payer; provides the link between provider and payer organizations, physicians and the community in the transition of patient care through the health care system.
*ASN required; Bachelor's degree preferred
*Current CT RN license necessary
*A minimum of 2 years of relevant clinical experience needed
*Worker's Compensation experience, specifically approving services and case management, essential
*Long term, acute care case management experience required
*Home Care and/or long term care experience preferred
