This is a PRN position. Coordinate care for a specialized group of patients needing assistance with chronic disease processes, symptom control, care goal decisions and end of life support. Support and advise the medical team and provide specialty care to patients and their families. Provide expert nursing direction in the development and delivery of palliative care to patients within the hospital and or outpt entities as required. Develop clinical practice standards of care and contribute to the training and education in this field of expertise. Promote the importance and value of the program in providing care for patients in need and support to their families
Ability to manage several projects and cases simultaneously.
Knowledge of Community Resources.
Knowledge of the uninsured and underserved populations in area
Ability to teach and educate clients on their level of understanding.
Basic computer knowledge.
Ability to communicate effectively, both verbally and in writing
Palliative/Hospice experience preferred.
Medical /Surgical experience as an RN
High school diploma or equivalent.
Graduate of accredited ADN or BSN program. BSN preferred.
Maintain Texas state RN licensure and provide organization with verification
