The Assistant Vice President (AVP) has strategic and operational oversight of the Care Management (CM) Department designed to provide seamless care management across the continuum of care for our patients through the coordination of the right care, at the right time, in the right place, by the right provider. This leadership role requires a highly collaborative, interdisciplinary approach to patient centered care coordination that is safe, reliable, and timely.
In this role you will be responsible for:
Setting the program's evolving vision, mission, strategic objectives and goals
Empowering the coordination of clinical care management activities, transitional planning and management of levels of care
Communicating and working closely with the Payor Relations Department on specific payor issues
Ensuring optimal utilization management and resource allocation
Prioritizing quality management, including patient's safety, risk management, performance improvement, and handling of delays or variances in care
Enabling patient's timely access to necessary services
Developing department directors and managers, and fostering a learning environment
Managing human resources and staff engagement
Overseeing the management of denials and appeals
Working closely with clinical leaders to manage length of stay and readmission reduction
Measuring the effectiveness of the Care Management program
If you possess the following qualifications, please apply immediately:
Healthcare related Master's degree and active RN licensure required
Minimum of five years clinical experience preferred
Minimum of five years executive or director level healthcare leadership experience preferred
Accredited Case Management (ACM) and/or case management certification (CMC/CCM) preferred
Experience with performance improvement and change management preferred
Case Management/Transition Management experience required
Training in healthcare quality (CPHQ), LEAN, Six-Sigma, or other performance improvement methodology preferred
