Provide case management and discharge planning services, including interacting, supporting, and consulting with physicians, social workers, and other disciplines to coordinate care. Duties also include evaluating severity of illness and intensity of service, as well as assessing patients' post-hospital needs.
PA licensed RN
At least 4 yrs of clinical experience
Case Management or Discharge Planning experience desired
Registered in Radiology Technology by the ARRT or an appropriate credentialing agency
Associate’s degree required; Bachelor’s in healthcare related field preferred
Strong human resources, communications and organization development skills and knowledge
Demonstrated ability to lead staff and faculty to ensure that the department’s long-term objectives are achieved
An understanding of physician, patient and technical operations
Excellent customer service skills
