CMCS is seeking an energetic, compassionate nurse to work with the clergy members and assist them in obtaining their highest level of health and wellness. This includes the promotion of wellness, health knowledge and condition management. The goals of the program are accomplished through visits and coaching eligible individuals in developing and maintaining self-management skills that lead to an optimum level of health: physical, mental, spiritual and emotional. Some travel is required.
REQUIREMENTS: Excellent verbal and written skills; Basic mathematical and statistical ability; Must be well organized; Basic computer knowledge; Knowledge of utilization review procedures/managed care; Knowledge of current health care practices and standards of care; Ability to work independently or with others (physicians, healthcare staff, etc.);
QUALIFICATIONS: Valid Indiana State Nurse’s License; Bachelor’s degree in Nursing or Licensed Practical Nurse with extensive Condition Management experience and skills; Health and Wellness Coaching experience preferred
EXPERIENCE/TRAINING: A minimum of 5 years in an acute medical/surgical environment; A minimum of 1 year of experience in managing patients with chronic conditions
