Responsible for directing all aspects of the Care Coordination department in order to ensure the most efficient and effective use of assigned resources in the provision of quality care coordination.
Develops and implements the philosophies, policies, procedures and goals for the Care Coordination Department.
Champions patient throughput initiatives to every area of the health system.
Provides after hours support to the AODs as it relates to admissions and transfers as well as discharge regulations.
Assists in the development, implementation and facilitation of hospital-wide strategic projects which impact Admission Opportunity, Length of Stay, Resource Utilization, Payor Reimbursement and Patient /Physician Satisfaction.
Prepares and evaluates monthly, quarterly and annual reports of the Department's functions.
Provides information regarding changes in Medicare regulations and documentation issues to physicians and others as needed.
Maintains Prospective Payment System, monthly case log and other files needed for peer review organization and specific needs of the hospital.
Analyzes physician utilization patterns, comparing to national and individual hospital standards. Communicates findings to Utilization Review and other appropriate individuals.
Discusses denial of coverage related to Utilization Review with the Director of Quality Management. Assist with on-site monitoring reviews by PRO, Blue Cross, outside review organizations and third-party payers.
Maintain a working relationship with local, state and federal agencies, recognizing the hospital's position in the community and its need for cooperation and assistance from such services.
MBA, MSN, MHA or other related Master’s degree OR;
BSN or BA/BS with 10 years of experience in the field including 5 years successful management/leadership experience
LICENSURE / CERTIFICATION
Current Florida RN Licensure
Case Management Certification Preferred
Minimum 5 years Care Coordination experience including 3 years management/ leadership experience
Demonstrated ability to function effectively in a leadership capacity
10+ years as Director of an acute care hospital or health system Care Coordination department preferred.
Demonstrated ability in a mentor role preferred
