Jewish Child & Family Services provides vital, individualized, results-driven, therapeutic and supportive services for thousands of children, adults and families of all backgrounds each year. Our Child Welfare department is currently seeking a Case Manager to provide vital case management services for foster care clients who are DCFS wards of the state.
Bachelor’s degree from an accredited program
At least one year of case management experience
Must be able to work evenings, weekends, and/or an on-call rotation based on client needs
As this position requires site visits and client transportation to locations which may or may not be accessible by public transportation, must be able to drive, have a valid driver’s license, vehicle insurance, and reliable access to a vehicle to travel in the Cook County/Chicago metropolitan area
Bachelor's degree in Social Work, Counseling, Marriage/Family Therapy or related Human Service focus
Knowledge, Skills and Abilities:
Carries a caseload comprised of DCFS wards of the state and their families
Performs a range of social work duties, including gathering and analysis of data and the formulation of treatment plans
Formulates understanding of clients’ situations, providing appropriate services to meet client/family needs
Transports clients and supervises the facilitation of sibling and parental visits, following DCFS policies
Attends juvenile court, collaborative conferences with DCFS, school staffing and other meetings that may impact the well-being of the client and the family
Advocates for clients’ best interest in school settings, at juvenile court and in any other systems which interface with the client and the family
Links the client and the families with community resources to help meet the treatment goals
Performs record keeping functions and other administrative duties are required by the Agency
Collaborates with other agency personnel, supervisor, appropriate specialists or consultants on specific case related problems
