The RN Case Manager will be responsible for planning, coordinating, providing and documenting care for a geographically assigned caseload of patients from day of admission through day of discharge or death. Other responsibilities will include, but are not limited to:
Performing admission assessment and evaluation of patient within 48 hours of referral or return to his/her residence
Developing a written plan of care for each patient
Assessing patient's condition, initiating plan of care, re-evaluating and updating plan of care as necessary
Utilizing appropriate pain management techniques and educating the patient and family regarding pain management
Demonstrating knowledge of medications and the correct administration based on age of the patient and patient's clinical position
Providing skilled nursing care by delivering competent, quality patient care in home
Obtaining all necessary physician orders and collaborating with physician as patient's needs dictate
Planning for discharge from the Agency with the patient and caregivers
Supervises Home Health Aides on an every other week basis
This position is a Full-Time Benefit Eligible position with visits to homes within a 30 mile radius from West Bend! Hours are Monday-Friday 8:00am-4:30pm with on-call one night per week required.
