Job Summary: Clinical Coordinator will: Work with the Executive Director and Program Managers to oversee operations of client programs. Provide clinical consultation, case planning, coordination and general supervision to the staff. Works directly as primary clinician, in the office and in the community, with agency clients who experience symptoms of major mental illness. Duties/Responsibilities Insures clients receive treatment services that are focused on psychiatric rehabilitation; Monitor referrals, admissions, continued stay and discharge criteria; Provides clinical direction and leadership with team; Directs crisis intervention efforts and intervenes as required; Evaluates staff, initiates and follows through on progressive discipline, if needed. Insures timely completion of reports and data on performance indicators, DMHAS standards, incident/accidents, etc. Ensure compliance with and adherence to regulatory standards including conformance with CARF, Department of Public Health, Medicaid and DMHAS standards Follow all Medicaid guidelines and ethical practices related to client care, service delivery and billing. Oversees completion of chart audits and ensures needed documentation. Reviews caseloads regularly to ensure appropriate utilization of services On call coverage during and after program hours as assigned Participates in committees to improve agency service deliver and quality improvement
Qualifications/Education required: •Demonstrate the ability to clearly communicate verbally and in written form. •Ability to plan, organize and prioritize work, while managing multiple deadlines. •Appropriate licenses as required by the State of Connecticut. •Excellent computer skills •Valid Driver’s License •Possession of a registered and insured vehicle during entire workday to transport yourself and/or clients for work related activities, including the transportation of Keystone House, Inc. clients to community sites throughout all neighborhoods.
Qualifications: Expert knowledge of severe and persistent psychiatric disabilities and psychiatric rehabilitation Knowledgeable on substance abuse treatment for individuals with severe and persistent metal illness. Excellent verbal and written communication skills Master’s in mental health field required; Must be licensed in the State of Connecticut – LCSW, LMFT, LPC, Ph.D.
