The HBPC NP functions as primary care provider for the assigned patients. The NP utilizes advanced practice nursing skills and knowledge in a relatively independent manner in areas of patient care, education, research and consultation. The NP collaborates with interdisciplinary healthcare team members on a local and national level to identify, implement, test, and export novel approaches to optimizing quality of life of older veterans. The HBPC NP conducts home visits to assess needs of patients and their caregivers. Performs comprehensive medical assessments/ evaluations in the patientsâ?? home setting with consultation with Medical Director, as needed. Performs medication reconciliation in home settings. Manages and treats medical conditions in the home setting, involving Medical Director as needed. Provides health education to patients and their caregivers. Refers patients to appropriate community or VA programs. Participates in weekly HBPC team meetings and provide innovative management ideas for patient case reviews.
