Works in conjunction with the Director of Case Management (DCM) to provide leadership, develop and standardize case management operations to include care coordination, discharge/transition planning and disease/injury management across all programs. While conducting this oversight, this position focuses on the fiscal aspects of health care management. Will support advocacy for patients and their families by promoting high standards of patient care, participating in program evaluation and development and other designated department related tasks as identified. Collaborates with inter and intradepartmental supervisory staff to ensure quality patient care, and functions within the scope of Shepherd Center's philosophy, policies and procedures. Promote and facilitate a multidisciplinary approach to coordinate care and plan operations among disciplines. Ensure adherence to standards of care consistent with current professional standards and consistent with Shepherd’s Mission, Vision and Values. Serves as a resource person of expertise within the CM department, Shepherd Center and the community. The essential function of this position is for providing support and assistance with the direct and indirect service activities of the Case Management staff as needed. This position provides direct service when needed as back up if a case manager is unavailable or needs additional support.
Clinical degree in allied health profession; Master’s degree preferred or eligible to sit for the Certification in Case Management (CCM, CMC or CRC)
