Take advantage of this opportunity to join our team as a Director of Case Management. The successful candidate will provide leadership for the Care Coordination/Clinical Resource Management Department by conducting strategic planning, directing, coordinating, and managing the Case Management function to facilitate the provision of seamless quality care in all aspects of patients' experiences.
This key position is also responsible for contributing to the mission and goals of the medical center by participating in the Hospital's as well as in the Johns Hopkins Medicine- wide strategic initiatives. This position will assure compliance with accreditation and other applicable federal, state, and local regulatory agencies, leadership and management responsibilities for the case management and utilization components of the case management team, and development of partnerships with other Hopkins affiliates and other external businesses.
Completion of a Master's degree in Nursing or a closely related field.
Current RN licensure in the state of Maryland along with minimum 10 years’ clinical practice.
Minimum 3-5 years in a Director role related to care management or care coordination.
Previous experience in building, leading, growing, and managing care coordination programs.
Prior experience in strategic planning and implementation of programs related to population health.
Certification in case management (CCM) preferred.
